Once completed, you will receive an email confirmation indicating that your application has been successfully submitted to both the US Customs & Border Protection and the Canadian Border Services Agency. How do I fill out my application form?

Before you begin filling out the application form, please first download it to your desktop to your device and then you can start filling it. To save the completed form, choose File > Save As and choose a location for your PDF, enter a new name (optional), and then tap Save.

Attach a PDF file (completed form) to e-mail: info@nexus-card.com

Click on “File” in the menu bar. Next, select “Send,” and then you’ll be presented with a range of options. You can choose to “E-mail Page as Attachment,” which will open a new email message with both the original file in its native format and a version of the file as a web page attached. Alternatively, you can opt for “E-mail Page as PDF,” which will open an email with a PDF version of the file attached, allowing for easy sharing and viewing. These options provide flexibility and convenience in how you share your files with others.
